See attached form: Client wants to make the fields in boxes "Owner Information", "Vessel Information" and "Insurance Information" a web form that would be associated with a user account. The information would get stored in a database in the back end. All of the fields in the "In-Water Slip Rental" section would be filled out by the admin in the back-end, and added to the user profile.

Once all of the information is filled out, the admin must be able to take all fields from the database and populate a printable form that would look exactly like this PDF. This would generate the yearly boat docking contract, and get emailed or mailed to the user to be signed. (A feature to email the contract right from the back-end would be ideal, or to "print" as PDF so it can be mailed)

The user needs to be able to log in at any time and update their information, as well as the admin. One of these contracts will need to be generated each year, since it is yearly docking. So, there needs to be a way to generate the contract and mark it as the "2011" or "2012" contract, and saved in back end.

Not too concerned about what it is written in, but would prefer php since this is hosted on apache. Needs well commented code, and plenty of testing to ensure it works properly.
