- Description: Need you to duplicate (copy) two web forms. Must be programed so that I receive a secure email when form is submitted that includes a database attachment in standard format which can be easily imported.

Need simple thank you / confirmation pages that address applicant BY NAME (merged data) when submitted.
Pages to be installed in existing PHP site that is slightly wider than the examples. Forms must behave exactly like example. (hover over question marks, copy address to co-borrower from borrower).
