PLEASE READ THIS POST PROPERLY BEFORE REPLYING.
NEEDED : Flash Expert with understanding of medical formula's and calculation.
We are currently developing a website for a client who specialises in selling training dvd's for people involved in stroke recovery.
( NB : A stroke is a burst blood vessel in the brain and is the 2nd leading cause of death worldwide )
One of the sections of their new website is called STROKE-O-METER which will enable anybody between the ages of 54 and 85 to assess their risk of stroke by entering a few personbal health details.
If you visit my client's old website by clicking on the following link ( http://www.stroke-education.com/RiskCalculator.do ) you will see their current / old stroke-o-meter in action.
Essentially my client wants a new interactive flash version of this Stroke-O-Meter to be created for use in their new website that functions in exactly the same way but has a professional new look ( skin ).
Unfortunately, we do not have the original files that the current risk-calculator ( at http://www.stroke-education.com/RiskCalculator.do ) were based upon which is why we need a new version built from scratch.
The dimensions of the total space for this to be created within is 935 x 581px but the dimensions of the of the new flash risk calculator can be smaller, possible around 745x418px.
The final files that must be supplied are the original .fla & the final functioning .swf
The brief given to me by my client containing all the medical formula and calculations needed are listed below and attached to this post on page 17 of the .pdf.
We are only interested in hearing from proven ' Flash Experts ' with a strong track record and understanding of medical formula's and calculation, who can complete this project before the end of Sept 2010.
In the first instance ONLY post links showing previous interactive flash calculators you have made ( or similar ).
WE ARE NOT INTERESTED IN SEEING ANY NON-RELATED DESIGNS FROM YOU.

To estimate a risk of stroke one needs to know the following:
Age of the person (currently, the algorithm can be used in people 55-84 years old only)
SBP (Systolic Blood Pressure) in mm Hg
Whether the person currently uses antihypertensive (blood pressure lowering) therapy (Hyp Rx): Yes or No
Whether the person has DM (diabetes mellitus): Yes or No
Whether the person is smoking cigaretes (Cigs): Yes or No
Whether the person had previously diagnosed cardiovascular disease (CVD): Yes or No
Whether the person has atrial fibrillation (AF): Yes or No
Whether the person has left ventricular hypertrophy (LVH)
Sex Man or Woman
Age Enter your age (must be within 55-84 years range)
Untreated Systolic Blood Pressure Answer if your blood pressure is untreated (enter your systolic blood pressure here).
Treated Systolic Blood Pressure Answer if your blood pressure is treated. (enter your systolic blood pressure here)
History of Diabetes Mellitus Yes or No
Smoke Cigarettes Do you currently smoke cigarets? Yes or No
Cardiovascular Disease If you have history of myocardial infarction [heart attack], angina pectoris, coronary insufficiency, intermittent claudication, or congestive heart failure.
History of Atrial Fibrillation Irregular heart beats: Yes or No
Left Ventricular Hypertrophy by electrocardiogramme: Yes or No
The formula to be used to estimate the risk of stroke in men is:
Risk = 0.0505 x Age + 0.014 x SBP = 0.3263 x Hyp Rx + 0.3384 x DM + 0.5147 x Cigs + 0.5195 x CVD + 0.6061 x AF + 0.8415 x LVH - 5.6770. The Risk is then exponentiated to the resulting value is called B. The final formula to estimate the risk of having a stroke within 10 years is as follows: Risk of developing stroke within 10 years = (1 - 0.9642) powered by value B (se above) x 100%.
The formula to be used to estimate the risk of stroke in women is:
Risk = 0.0505 x Age + 0.014 x SBP = 0.3263 x Hyp Rx + 0.3384 x DM + 0.5147 x Cigs + 0.5195 x CVD + 0.6061 x AF + 0.8415 x LVH - 7.5766. The Risk is then exponentiated to the resulting value is called B. The final formula to estimate the risk of having a stroke within 10 years is as follows: Risk of developing stroke within 10 years = (1 - 0.9642) powered by value B (se above) x 100%.
The interpretation of the risk should be given in the following format: On average, within a 10 year period X out of every 100 people with the risk factors you have outlined above will have a stroke. If we base the calculation solely on your sex and age group, then the risk of stroke within 10 years is X%. Thus, your risk of stroke is approximately X times greater than that of someone your age that does not have other contributing risk factors. (NB. See Table 4 in the attached article for the risk of a stroke for a given person based on age and sex of that person; the data from that table should be used when concluding how the risk of stroke of a given person is different (similar or greater and if greater then how much greater) than that of someone of his/her age who does not have other contributing risk factors).
Please feel free to change the values in the calculator to see how it affects your risk outcome.
Please note that the risk of having a stroke for Europeans of African descent, Americans of African descent, Maori, and New Zealand Pacific Islanders is approximately 30% higher than shown here.
In the formula above, SBP is systolic blood pressure, Hyp Rx is use of antihypertensive therapy, DM is presence of diabetes mellitus, Cigs is cigarette smoking, CVD is previously diagnosed coronary heart disease, cardiac failure, or intermittent claudication, AF is presence of atrial fibrillation, and LVH is left ventricular hypertrophy by electrocardiogramme.
