Your job is to make a Amazon S3 upload form for Joomla.

The (registered) user must be able to upload a file of my choice of size of any file type of my choosing through an attractive form with a browse button.

The user may upload up to 5 files at once (extra 4 dropping down with a upload more... button or something).

There needs to be a 'description' sort of form as well, which is sent in an email later once the upload is successful.

Once the user uploads the file must upload to a specified bucket in my S3 account, you may use either of these options, whichever is easiest to code;
1) Folder is created for each user based on their username, with no spaces or funny characters.
2) File is prefaced with the above information i.e username123-file01.zip

There must be a progress bar while the upload is happening.

The user must be notified the upload was successful & an email MUST be sent with the upload time, size, filename, description & username to a specified email (mine).

You may use flash, php, ajax - whatever.
