The Excel file attached includes approx. 21,000 rows each including data on doctors and dentists in the US. The data requires normalization so that:

1.) All address fields include only street address 1, street address 2 and no longer include c/o, place names, etc.. In short, they need to be geo-locatable addresses when combined with the city, state and zip code fields and include no extra data.

2.) All zip fields should include all 5 characters, including leading zeros when necessary and should NOT include any the hyphen or plus four characters

The file should be returned in Excell 2008 format.
