- Description: I need this form made for my insurance website. I need somebody that can copy the exact form field, form flow, and form functionality of this form at https://www.netquote.com/quotes/home-insurance.aspx and integrate it/customize the design into my website. Once again the link is:

This form above will require AJAX implementation, so you will have to browse through each form field value to see all the different form field combinations. There will be lots of data table creation and php coding from scratch because I don't have all data available. The programmer will also need to scout all the data for database creation. I do not have PHP code files yet so the programmer will need to make them available for ajax calls as well. In addition, ajax calls and ajax data checking are advanced features so I expect the person I hire to be very familiar with these features.
