We are looking for an iPhone app through which we can create/design a small survey (4-5 question) and host it on our WebServer (LAMP). Further this survey should be sent to few people in my organization who will have a client app installed on their iPhone. They will use this client app to download the survey, fill the responses and submit it back to my survey server.
We also need an Admin tool (iPhone and PC) that will be used to view the data/statistics of people who have submitted the data.
It will be great if we can also get a simulation environment for iPhone App to visualize the survey created before deploying/sending it to the people.
Please reply with total project effort and costing.
