- Description: We are looking for a programmer to convert or word document into an online form. Form will have where client fills out and submits and we and client that fills out forms is emailed a copy.
Word document id currently about 4 pages. Question for form designer not sure how this will work. Is there a way to make sure client is filling out form. Example form will be an agreement of our services. We were thinking of having client print and fax but most client will not take the time to print. Our reason for asking is client can say that they never or someone else submitted form.
Please send any sample forms that are relevant to project, we do not need links to your portfolio unless they are forms similar to project request.
Also note we are running site on linux server.
