Hi!
I have a website that currently uses a simple form that does not work regularly any more.
1) The new form will be implemented into a CGI script so it will have some data taken out of a database. These data (see attachment) are inserted like this: [data]
2) All fields should be mandatory
3) Some spam savety should be in it (maybe a 2 letter chapka)
Below you will find our current form
4) 3 mails need to be sent: one to the customer who filled out the form, one to the name and email takan from DB, one to us (same as the one from DB).
