On my existing web page, I need to add a form to where visitors can enter their email address and first name. Then hit the submit button and be redirected to another existing web page.

The email address and first name needs to be sent to my Constant Contact list name Open Source Project.

I have the instructions and sample code from Constant Contact to set-up.

This is an urgent project.
