I need to have a website created, that will allow users (doctors) to create their own website. My website will then act as a network for doctors and their patients. Doctors will be able to create websites, and patients will be able to search for doctors by city and state, zip code, medical specialty, etc.

I can design the "index" web page, I just need a programmer for the following:

1. Upon payment, the user is allowed to create an account (username and password protected). I would like the user name to become their web address (for example: if the username is "username1" then..."http://www.mysite.com/username1" is their new
web address). When they create an account, they will complete several fields (full name, user name, password, medical specialty, address, city, state, zip, etc).


2. Once logged in, the user can design a multi-paged website (up to 8 pages): template selection, ability to edit text
(color, size, font, hyperlink, etc) and add images (their own, or select from images in my library), add/remove buttons, implement a "contact us" form, etc, etc. I would like to be able to add to the library of images and Flash that each user can select from.

3. I need an administrative login with the following:
a) I need to be able to view a list of all accounts (with details: username, full name, email address, etc), and I need the ability to edit, delete and suspend any account. I need the ability to search my database of accounts by account name, email address, membership join month, medical specialty, zip code, etc.

b) This will be a subscription service (annual). I need to generate a Monthly report that will tell me all accounts that will expire in the current month. I would like each account on this list to automatically receive an email directing them to my payment page.


4. There will be a "Search" page that will allow patients to search for doctors by zip code, city, state, name, medical specialty, etc (this will search the database of accounts). Would it be possible to also display the doctor's location on a map (this can pop-up in a new window if "Map It" is clicked)?
