- Description: I have the layout of an info request form. There will be a couple of changes to the fields/text.
I need it to be turned into a web based form (rewritten with clean code) with a php script to receive the submission and create a nicely formatted email to a designated email adress. I would like another email sent to the person who submitted it. I don't care if you use a free existing script as long as it is secure, and the variables are clearly defined and easy to change.
The script should be secure, with CAPTCHA to prevent spam.
The form should have javascript validation to prevent incomplete submissions, as the end client determines.
The sample form is attached.
This will need to be created and tested on your own development server and delivered to me as a zip file.
