I need a report that pulls all orders by product. The report will be used as a class roster, and must include customer name, address, phone number, and email address.

If the customer has purchased more than one class, I'll need blank space to fill in the information for the additional person taking the class.

At the end of each product list, I'll need an attendence sheet. This will just be a the customer's name (maybe email address and phone number) and a grid so a teacher could check off attendance. As above, if the customer has purchased more than one, I'll need additional space to add the additional person's name.

I will provide a mock up of the report I want.

I would then like the each of the products to have a mailing list created for them. This should be setup so that the teachers could login and email the entire class.
