- Description: I need to renovate my website, www.NYDentalSpa.com . I posted this project before and selected a programmer, but after much time working with him he admitted that “he was more of a designer than a programmer” and could not complete my project as requested. Sidenote, he also took much longer (7 days vs. 1 day). I want a programmer who can do the following in a respected amount of time. I will work with them on it to make sure it is the best.
1) I want to use a preexisting template. I like this one: **NOTE: This website incorporates flash, I want the flash version.**

The programmer must be able to edit the template. For ex, change the menu color to a brighter red, add images, add logo, change color schemes.
2) I will supply all the written material to the website. This is already completed and will be sent to programmer as soon as a programmer is selected.
3) I need an APPOINTMENT FORM, not a contact us page. This is the main reason as to why my previous project failed. The programmer could not supply an appointment form. The appointment form would have the following information:
First Name
Last Name
Primary Phone
Secondary Phone
Email Address
Date: Calendar Box Opens.
Time: Drop Down Menu from 11AM-7PM
Reason for visit: COMMENT BOX.
4) I will supply a logo, programmer must be able to edit it so it can fit the website appropriately.
5) Be communicative! I want a programmer who I can speak with online on a messenger or via an email address, not just through scriptlance.
Also, I would like it to be easy for me to go in and manage the website. By manage I mean edit the text information if needed.
Nevermind the programming and database options I am not knowledgeable in this area. But I do want the website to be SEO.
