I would like a simple script written (preferably php) that will allow clients to fill out a form, browse, upload and attach files as necessary, and then upload the form and attachments to a newly-created folder on a server.
The form subject and folder name will then be emailed automatically.
I will provide the form html, thus saving you time. No styling etc is needed.
