Hi I am looking for a programmer to create a simple contact form or even don't create it from scratch you can modify phpmailer or anyscript.

what I want is a form similar to this one http://tinyurl.com/form2313 I want users to be able to download files to me but what I want is I want to copy of this script.

the first one will send the file as an attachement directly to my email
the second one is upload it to a specific directory in my website (for example mysite.com/upload) and send the link for the file to my email so I can download it from, once somene upload a file I want the file to be renamed with the date of upload and the user's name (example: user_11012008_filename), I want to be able to set the maximum file size as well as the allowed extensions.

once the upload done I want to specify a thank you message in the same page I don't want to direct people to a thank you page. I want the php file to be clear and you have to comment where I can edit in the file (for eample to specify the allowed file types, or changing the thank you message). also I want to be able a required fields and if someone click on submit without the required field a message will tell him that required field is missing (email and name are required) I know enough about php and I found tons of forms online but I don't want to spend time working on it and customize it so do it for me :)
