I need a multi combo search form created which uses my database as it's options. Something like the advanced search available here:

Below are the current search fields I would like to include on my form.
category
sub-category
state/region
make
model/version
keyword(s)* optional, requires database table work
price limit (predetermined, use any example for now)
sort listings (options would include date, price, location etc)
Here is a list of fields available on my database (note they may be named something else to form):
id
title
price
description
year
model
type
subtype
make
suburb
state
postcode
Below is the output which needs to be shown after a search is generated:
title
price
id
suburb
state
I would like for someone to create the ajax/javascript aspect of the form and also the php/mysql driven output which it creates... the output needs to be paginated (I have an example script you can modify to speed things up) Any takers, how much?
I also need a submission form which includes images upload and manipulation into the above database/server (I have an example script for this too that you can modify to speed things up). In addition I need an authorize.net payment gateway or similar (not paypal)... would be keen to work with someone who can do both.
