I need 4 total forms created. 2 using php/mysql to store data and 2 simple questionnaires that email directly to the site owner.
Form 1: DESIGN QUOTE FORM: approximately 20-25 form objects to fill out including contact info, area to input dimensions for a quote, radial buttons, etc. Will submit and store in a mysql database that the site owner can login and view data. Needs to be very simple. Will need to email site owner when form is received telling them to login. This will also need the capabilities of uploading up to 4 items stored on the web server, they are files for the site owner to view for the quote request.
Form 2: Simple contact form with 5-6 fields, which will email directly to site owner.
Form 3: 10 question form. Will ask a question and have a field for the visitor to fill out to answer the questions. Will need area for visitor to fill in contact info as well. Will submit by email to site owner.
Form 4: Same as above but with 16 questions.
These forms will be used on different sites so they will need to be self-contained for each of 4 forms and not using the same processing script for all 4.
