PHP & MySql project: I need a DB and code to capture email address, name, IP, date/time stamp, source URL, and an "Ad Code" (set by Admin), from a form submit originating from both email and web page. The form will require first name and email and when submitted by the individual the script will have to capture all other required fields. ALSO, the DB must check for duplicate email and/or IP and identify ALL dupes in data output. Once submitted and captured in DB, script must redirect to a specified URL. Report output all of data captured in Excel spreadsheet, within a specified date range, and/or by specified ad code thru simple Admin Panel.
All must be installed on our server and tested. Must also provide code, html only, no javascript, for webpage AND email which places the form and all code required on webpage and/or in HTML email.
We have a high rating on Scriptlance and will treat you fairly and honestly, but you must be able to perform the task. We are based in the U.S.A. and our office hours are 7am - 7 PM CST and we encourage the selected winner of this project to communicate with us.
You must agree to revisions as required by us to make the project 100% to our satisfaction. Payment will be made once the job is completed to our satisfaction. Complete testing of the application will be required before payment is made. We will only escrow for providers who have a rating higher than ours.
