I hired a programmer for a project and he was in a car accident. I need someone to finish the project. He got a bunch of things done, but it needs to be finished. You'll have to go through everything and see what needs to be be done, uploaded, and verified.

Some of the graphics in the above zip file are a little messed up. Below are the original files, including how the forms should look, with the graphics that should be used. The forms need to be exactly how the layout shows. It is pixel-specific.

This really shouldn't be that difficult. Much of it is already completed. My budget for this is no more than $35. So much is already done.I will compromise too... If you need to use a simple table, then do so if it will make the project easier and faster.
