	Description:	I hired a programmer for a project and he was in a car accident. I need someone to finish the project. He got a bunch of things done, but it needs to be finished. You'll have to go through everything and see what needs to be be done, uploaded, and verified. Below are the original files, including how the forms should look, with the graphics that should be used. The forms need to be exactly how the layout shows.
