Need a small database script to display pricing for a chauffeur company

Select starting point:
Select Destination:
Select Time:
Display to show:
Miles:
Drive time:
Cost:
For consideration:
I need an option / function to change the pricing. Pricing is calculated miles x cost / mile = total price ie 20miles x £1.50 = £30.00
I also need additional functionality to enter in new starting points (towns/cities)
Please supply quotes for an empty database and one with data entered.
