Summary: Have existing forms that are full of old HTML, they need to be rewritten, given a form processor, made table-less so I can just copy them into a new site design.

I need someone to take approximately 7 simple contact forms:

1.) make them semantic and accessible (e.g. use proper validatable tags)
2.) apply css styling like what is found here http://lipidity.com/fancy-form/ ** - I want to use this styling for the checkboxes and radio buttons - I will need assistance in making sure and text-areas are similar - I can do the graphics - needs to fuction nicely for common browsers and degrade nicely
3.) I want the CSS submit button to have a click or hover effect
4.) make them all table-less - many will be two-column
5.) write the sender's name and email and form title to a text file
6.) check forms for required fields and give them a notice of missed fields before submitting
7.) make or use an existing form procesor & use ajax so that they send without having to reload the page (give a nice little "your message was sent" when sent)
8.) format the data nicely going to the recipient
9.) send a nice reply to sender from a text-file
10.) on the 3-4 of the forms, there will be a Date Select - a little javascript, dhtml, or ajax calendar should be used for date select for the primary requested time and the secondary time - I want something simple, clean that pops-up in a div, not a separate window
11.) I want to get rid of any drop-downs that are the existing forms. Want to turn them into checkboxes or radio buttons, whichever is appropriate
12.) I did a background graphic for the text-area. If I need to change it, let me know.
13.) thoroughly test the forms and processor for me


None of the forms are really complicated. Probably 8-15 text fields, 4-8 checkboxes and radio buttons, and 1-2 text areas each. Most have HTML that must be stripped.

Attached I have included a sample of how it needs to be laid out. This is a size-accurate example. Column width is exact. I will provide any and all graphics you need.

I need someone who is going to take this project and run with it. Someone who can handle all of the little details.
