Description: |
We need an online form for vendors applying to be a part of our shopping event. The form needs to serve two purposes in one:
One - Custom Form:
- Collect data such as in a traditional contact form. This form needs to have text boxes, radio buttons, drop down buttons, privacy/liability/legal disclaimer boxes, etc...
- The data then needs to be captured into a database with as little management on our part as possible.
- We want this database to be updated every time someone submits a new form - whether it be alphabetical or some other way - similar to an excel sheet format.
- We also need email notification to us as the recipients and to the applicant as form being received.
Two - Custom File and Image Upload Form:
- Applicants using this form will need to upload as many as 10 images/files.
- We have the ability to store them on our server (yahoo small business), but we need them to be organized into their own database. Similar to File Chucker by Encodable.com, but on our own server.
- For example, an applicant would create their own file directory (artist one) and all of her images would be stored in that file.

- we would need this information to be securely transmitted.

* We will provide you with a template of how we want it laid out, but will want to be able to go in and change the order of the items on the form, as well as the type of form elements they are
