Description:	I need a 4 step order form replicated ASAP. Here is the example order form. https://www.mailtrust.com/noteworthy/signup It must have built in error checking for each field. User should be able to go back and forth between pages without receiving those "post" errors within the browser. This order form must work very well. It must be 100% bug free. You do not have to do the web site design. This has already been completed. Yet, you do have to integrate our design into the order form. I will send you all the files you may need. Below is description of project working off of the above example. Step 1: Pricing. Must have all this same javascript pricing done on the fly. Don't steal their code. Add an extra Length of Term field to this section. Please do this with radio buttons, and list different pricing amounts for each term. 14 day free trial 1 month 6 months w/ 10% discount 12 months w 20% discount Step 2. Choose domain name. Make sure the code to check for domain name availability is not only fast but accurate. Their order form has an option for them to manage dns. Do not include this step. Include everything else. Step 3. User Info. All same fields, but do not include security question. Step 4. Billing INfo. All same fields. Step 5. Success, Thank You page. Display a Thank you Page. Lowercase all variables, including TOTAL and save results to a mysql database and a txt log file in pipe delimited format.
