I need a SCRIPT for an INPUT FORM on my site.
The INPUT FORM basically asks my site's visitors to submit their name, email address, age, gender, and questions to me. The information will then be emailed to me at the email address I specify.
The INPUT FORM script should have a security feature that prevents people from spamming me. Basically the script should ensure that is is a LIVE person entering their information to the INPUT FORM and not a program that aims at spamming me via the INPUT FORM. For example, the script should ask people to perform a simple math calculation before clicking the submit button.
The script should be simple enough so that I can basically copy-and-paste the script to different html pages throughout my site.
Do NOT bid unless you have FIRST PMB and let me know that you are CONFIDENT that you can do this project. Anyone programmer who bids on this project without first PMB me will be ignored.

This should be a VERY easy project if you know basic html. So be reasonable with your bid.
